Service Description
The Sierra Elder Wellness Program is a field-based program providing outpatient mental health services for the growing older adult population, ages 55 years and older. Services include outreach, mental health rehabilitation and therapy, co-occurring disorders services, psychiatric medication and follow up services, housing support, case management and other support as identified.
To Qualify
Service To Qualify
Adults 55 years and older needing the highest level of service support in the community. Types of issues include frequent emergency service use, multiple or frequent inpatient care, mental health or functional impairments resulting in frequent crises, co-occurring medical or substance use issues, and return to the community from institutional placement or care. Assessment for need of this level of service is completed by Adult Mental Health Access Team.
Cost
Service Cost
Cost is based on program specifications, MediCal coverage or other public insurance/resources, and the individual’s ability to pay. The Adult Mental Health Access Team will assist in determining the individual’s share of cost as applicable.
