EMT Recertification Current or within 6 Months Documents


INSTRUCTIONS FOR CURRENT EMT RECERTIFICATION - CURRENT EMTs OR WITHIN SIX MONTHS
To be recertified as an EMT in the State of California by Sacramento County  you must upload the following:
 
  1. Twenty-four (24) hours of Continuing Education hours or an approved EMT Refresher Course.
  2. Skills competency verification form, EMSA-SCV (08/10), with original signatures. This form MUST be the current 08/10 version to be accepted.
  3. Drivers License or other Government ID (Agency ID will not be accepted).
  4. Current and valid CPR for the Healthcare Provider card or equivalent.
  5. Current or expired (within 6 months) California State EMT card.

If you were certified previously by a different LEMSA and are filing an application with Sacramento county for the first time, you must also upload:

     1. Department of Justice (DOJ) and Federal Bureau of Investigations (FBI) Live Scan background check.
