Clinic Services
If you need medical attention call the County Operated Clinic listed on this web-site. When you arrive for your appointment, your eligibility will be determined and, if eligible, arrangements may be made for you to see medical personnel. In the event specialist services are recommended, our staff will schedule the appointment(s) as medically indicated. You will be notified of the time, date, and location of the specialty appointment(s).

ANY SERVICE YOU RECEIVE OUTSIDE THE PRIMARY CARE CLINIC MUST BE AUTHORIZED IN ADVANCE BY CMISP CASE MANAGEMENT.

Emergency Inpatient Services
If you have been admitted through the emergency room to a Sacramento County hospital, you may request an eligibility determination by the Department of Human Assistance (DHA).  Once eligible, our staff will take care of scheduling all your further medical appointments and notify you of the time, date, and location.

Emergency Room Services
If you are seen in a Sacramento County emergency room that provides outpatient emergency services, you have the right to request an eligibility determination.  The hospital will give you a referral from instructing you how to apply for CMISP.  Please CAREFULLY READ and follow instructions on the referral form given to you by the hospital staff.

You must be seen in the hospital emergency room of a Sacramento County hospital and not a “prompt care” or convenient care” in order to be considered for CMISP review of your services.

Eligibility for CMISP must first be established before any of your emergency room services can be reviewed. The Department of Human Assistance CMISP Eligibility Unit is responsible for CMISP eligibility determination and qualification to receive medical services.

ONLY TREATMENT OF CONDITIONS WHICH WOULD LEAD TO DISABILITY OR DEATH IF NOT IMMEDIATELY DIAGNOSED AND TREATED WILL BE APPROVED FOR PAYMENT. IN CASE OF EMERGENCY PLEASE PROCEED TO CLOSEST HOSPITAL.

The County Operated Clinic offers medical care for non-emergency conditions.

Ask the hospital staff if the services you are seeking may be provided through a contract between the hospital and the Sacramento County CMIS Program.
