Refugee Preventive Health Program


Description

Under the Sacramento County Chronic Disease Prevention Program, the Refugee Preventive Health Program (RPHP) assists newly arrived refugees in achieving self sufficiency by becoming and staying healthy through prevention, and management of chronic conditions.  The RPHP promotes overall well being, behavior changes for healthy living, and improved self-management of cardiovascular risk factors.

We offer the following services:
 
  • Linguistically and culturally appropriate chronic disease prevention counseling addressing heart disease, nutrition, exercise, diabetes, and other topics.
  • Program promotion and networking with local refugee populations through participation in health fairs and coalition meetings.
Program staff
The Chronic Disease Prevention Program is staffed by a Program Coordinator, Health Educators, and/or Health Education Assistants.
 

To Qualify

​Newly arrived refugees (within one year) referred by the Sacramento County Refugee Health Clinic are eligible for services.
